Field-Based NP/PA Clinician Home Health & Care

Elevance Health is seeking an Advanced Practice Clinician for field-based care, supporting the Norfolk/Virginia Beach area. Responsibilities include providing services in home settings, developing clinical care plans for adult patients, and educating patients about their health. Candidates must hold an MS in Nursing and an active Nurse Practitioner or Physician Assistant license. Preferred qualifications include 2 years of experience in home-based services and familiarity with Electronic Medical Records.
