Roswell Park Cancer Institute

Roswell Park Comprehensive Cancer Center in Buffalo, NY is seeking a Case Manager to coordinate patient care from admission through discharge and post-hospital follow-up. You'll collaborate with patients, families, physicians, interdisciplinary care teams, and third-party payers to ensure appropriate levels of care, seamless transitions, exceptional outcomes, and use of resources.
