Youth Care Coordinator II - Family & Community Care
Hillside Family of Agencies
A community services organization in Syracuse seeks a Care Coordinator II to provide high-quality psychosocial care for youth and families. Responsibilities include coordinating care, assessing individual needs, and delivering psychoeducational training. A Bachelor’s degree and relevant experience are required, with opportunities for mentoring and project leadership. Applicants must have a clean NYS driver's license and may need CPR certification.
