Discharge PlannerThe successful candidate will have the ability to assess, evaluate, and drive effective discharge plans. Responsibilities include coordinating services to ensure patients receive appropriate levels of care and ensuring treatment plans are initiated, evaluated, and re-evaluated daily. Key tasks involve monitoring length of stay, identifying barriers to discharge, and adhering to regulatory guidelines.ScheduleMonday to FridayWeekend: NoneCall: NoneFloating: As needed within facility/scope of practiceMinimum ExperienceCandidates must have at least 3 years of relevant experience. First-time travelers are not accepted.Clinical ScreeningAll candidates will undergo a clinical review to determine fit. If the candidate meets clinical criteria and aligns with organizational guidelines, their application will be forwarded to the hiring manager for final consideration.Requested Time Off (RTO)Maximum RTO: 7 days (discuss during interview)No RTO requests during the month of SeptemberHolidaysIf one major holiday falls within the assignment period, the travel professional is expected to work it. For two major holidays, both must be worked. If three major holidays occur, the traveler is expected to work at least two.
