Nurse Practitioner, Evenings, Nights and Weekends Per Diem

Job Summary Resource Nights & Weekend (ReNW) Nurse Practitioner (NP) – fully remote virtual serious illness care management for patients and families across Chapters Health System continuum of care, including home‑based, facility‑based, inpatient hospice, pediatric hospice, palliative care, and PACE. Responsibilities Provide fully remote virtual serious illness care management throughout the continuum of care. Consult with medical staff and interdisciplinary team to ensure optimal patient care. Perform virtual assessment of patient/family/caregiver needs. Identify, manage, order, and prescribe medication and equipment as appropriate. Adjust and support the plan of care in accordance with Chapters policies, procedures, and nurse practice act guidelines. Assess caregiver ability to meet the patient’s immediate needs and confidence in the patient’s plan of care. Provide education to increase caregiver confidence as necessary. Support staff with answering clinical questions, entering orders and prescriptions, and other clinical needs of patients and families. Maintain access to all electronic medical records (EMR), electronic prescribing systems, and required websites and applications. Document all interactions in the EMR in a timely manner. Collaborate with the primary team to ensure continuity of care. Support discharge planning, including prescribing medications, entering discharge orders and instructions. Participate in Quality Assurance/Performance Improvement (QAPI) activities as needed, including medical record reviews and peer reviews. Seek continuous improvement of care practices to assure compliance with regulatory and accreditation standards. Maintain ongoing knowledge of research and trends in Hospice and Palliative Medicine and participate in continuing education required for licensure. Obtain and maintain hospital/facility credentialing as required. Communicate and collaborate with team members, patients/families/caregivers, partnering providers, and staff within HIPAA guidelines. Provide patient and family education to reduce anxiety and facilitate high‑quality care. Serve as a model of exceptional customer service through effective and timely communication. Qualifications Minimum 3 years recent clinical experience, including substantial hospice, palliative care, and/or acute inpatient care. Master’s or Doctorate degree in Nursing from an accredited Family or Adult Nurse Practitioner Program. National certification, licensure, and prescriptive authority in home state and additional states where care is provided; maintain for all required jurisdictions. Basic Life Support certificate and compact RN license in home state by date of hire. Ability to work independently and as part of a team, with strong interpersonal, organizational, and problem‑solving skills. Physical ability to perform essential functions and mental/emotional resilience to work with death/dying. Proficiency with patient health information systems, telephony systems, Microsoft Office (Word, Excel, PowerPoint). Experience with HIPAA compliance and remote care delivery. Schedule Shifts may include nights, evenings, weekends, and/or holidays based on business needs. Compensation $52.50 per hour.
