We are dedicated to our mission that all patients and families entrusted to our care will be treated with the highest level of compassion, respect, and dignity.Our CultureOur culture is cultivated using the following values:Integrity: We are honest and professional.Trust: We count on each other.Excellence: We strive to always do our best and look for ways to improve and excel.Accountability: We accept responsibility for our actions, attitudes, and mistakes.Mutual Respect: We treat others the way we want to be treated.QualificationsAn Average Day: (Includes, but not limited to)Provide direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life careImplement current nursing practice following a comprehensive assessment and the plan of careProvide accurate and timely documentation of patient services to reflect the plan of careAssess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of careParticipate in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group; examples include hospice aide, OT, PT, SW, ST, Dietician or Clinical SupervisorUse and prepare equipment and supplies effectively, adhering to aseptic techniqueProvide appropriate pain/symptom managementEvaluate patients response to treatments/medicationsParticipate in personal, professional growth and developmentParticipate in organization's quality assessment performance improvement teams and activitiesPerform other duties as assigned by the registered nurseAll other duties as assignedRequirements:Must currently be licensed as an LPN/LVN in the StateMust have three (3) years nursing experience, community health/hospice or medical/surgical experience is preferredMust understand the philosophy of hospice concept and understand needs of the terminally illMust be flexible in work hoursMust demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programsJoin a Team that embraces the reverence of life!EEOC StatementBristol Hospice is an equal-opportunity employer.
