We are seeking a compassionate and driven Registered Nurse (RN) to join our Continuous Care team—providing one-on-one support when patients and families need it most. in the Broward County area while providing exceptional care on the following schedule: Per Diem 2 days 7am - 7:30pm and weekend availability.

What You'll Do

As a Continuous Care RN, you'll be at the heart of meaningful care—bringing comfort, expertise, and reassurance directly to patients and families.

• Deliver exceptional, compassionate care aligned with our mission and values
• Assess and identify patient and family needs with empathy and clinical expertise
• Coordinate individualized care under the direction of the primary physician
• Collaborate as a key member of the Interdisciplinary Group (IDG)
• Implement and support personalized plans of care focused on comfort and quality of life
• Utilize monitoring, intervention, and teaching skills to enhance patient well-being
• Provide reassurance, education, and emotional support to families
• Ensure a consistently high level of personalized, engaging service
• Address concerns promptly to ensure complete patient and family satisfaction
• Uphold the highest standards of confidentiality and respect

What You'll Need

• Registered Nurse currently licensed in the State of Florida.
• BSN preferred
• Hospice certification preferred.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus. Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license.
