Job Title: Occupational Therapist (Full-Time)
Location: Sulphur Springs, TX
Facility Type: Skilled Nursing Facility (SNF)
Synergy Care is seeking a Full-Time Occupational Therapist (OT) for Carriage House Manor, a skilled nursing facility in Sulphur Springs, TX. This position offers a supportive team environment and an excellent benefits package.
Responsibilities:- Evaluate and treat patients with physical, emotional, or developmental challenges
- Develop individualized treatment plans
- Help patients improve daily living and functional skills
- Collaborate with interdisciplinary team members
- Maintain accurate documentation
- 12 paid days off + 3 sick days

- Degree from an accredited Occupational Therapy program
- NBCOT certification (or eligible within 6 months)
