As a Nurse Practitioner (NP) or Physician Assistant (PA) at Centerwell Senior Primary Care, you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care.

You will work on site at our Centerwell Poinciana. You will report to our associate medical director.

You'll be the provider of the day seeing walk-in and some hospital follow-up patients and referrals.

Use your skills to make an impact

What we're looking for:

• Master's Degree as a nurse practitioner or completion of a physician assistant program with board certification.
• Active, unrestricted NP or PA license in the state of Florida.
• Two years of experience in value-based care, geriatric or primary care, managing adults aged 65+ with chronic conditions.
• Proficiency using an EMR.
• If selected for this role, you will be required to be screened for TB.
• Fluent in Spanish and English.

• Urgent care or emergency medicine experience.
• Experience in Medicare Advantage settings.
• Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers.
• Familiarity with HEDIS quality indicators and Medicare guidelines.

Scheduled Weekly Hours: 40

Pay Range: $123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
