Description

• Conduct evaluations of patients in inpatient or outpatient environments and create personalized treatment plans
• Implement physical therapy interventions designed to enhance mobility, strength, balance, and overall function
• Collaborate closely with nurses, doctors, case managers, and fellow rehabilitation professionals to maximize positive outcomes
• Educate patients and families on rehabilitation objectives, discharge procedures, and at-home exercises
• Document assessments, interventions, and patient progress in line with facility and regulatory standards
• Actively participate in care team rounds and interdisciplinary meetings to ensure seamless and coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Specialized certifications (such as in neurology or geriatrics) are advantageous, though not mandatory

Skills

• Prior experience in the field is highly preferred
• Capability to treat patients with complex medical and mobility challenges
• Excellent communication and collaboration abilities
