We are proud to be a mission-driven, values-based organization and a Certified Great Place to Work.As a Nurse Care Manager, you will play a critical role in BHN's Complex Care Management (CCM) Program, helping individuals with complex medical, behavioral health, and substance use needs navigate healthcare systems and maintain stability in the community. This role is ideal for nurses who want to move beyond bedside care and focus on whole-person care, care coordination, and long-term impact.Responsibilities include:Serve as the clinical lead within an interdisciplinary care team, helping guide treatment and care coordinationConduct comprehensive medical and behavioral health assessmentsProvide ongoing medical monitoring and clinical support for individuals with complex health needsDevelop person-centered care plans that reflect the goals and needs of each individual servedCoordinate care with primary care providers, psychiatrists, specialists, hospitals, and community agenciesSupport individuals after hospitalizations, emergency department visits, detox stays, or other inpatient careProvide medication reconciliation, education, and adherence supportAddress social determinants of health such as housing, transportation, food access, and barriers to careLead interdisciplinary care team meetings to ensure coordinated treatment planningProvide crisis support and stabilization interventions when neededConduct outreach visits in homes, hospitals, shelters, clinics, and other community locationsHelp individuals transition from higher levels of care back into stable community livingQualifications include:Registered Nurse (RN) licensure requiredBachelor's Degree preferredMinimum 2 years of healthcare or human services experience requiredExperience with behavioral health, substance use, care management, or community health strongly preferredPassionate about community-based care and improving health outcomes for vulnerable populationsStrong clinical assessment and care coordination skillsExcellent communication and relationship-building abilitiesProficiency with electronic health records and standard computer systemsValid Massachusetts driver's license and reliable vehicle requiredVehicle must be registered and inspected in accordance with 540 CMR 4.00What We Offer:Competitive salary based on experience Comprehensive benefits including health, dental, vision, and (403b) retirement plan.
