Nurse Case Manager - Baltimore, MD

The Surprising Truth About Case Management - Paradigm Watch this short video for a brief introduction to the role of a nurse case manager at Paradigm. A Field Nurse Case Manager is hired to support injured workers throughout Baltimore and nearby communities , including Phoenix, Towson, Aberdeen, Middle River, and other surrounding areas. A Field Case Manager role is a hybrid position. The candidate must be located in the Baltimore area due to regular local travel (80% of the time) for in-person patient appointments. You must be willing to travel and have a compact RN license in the state of MD. This individual is responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carriers. Responsibilities Provide medical case management for workers’ compensation cases, including assessment, planning, coordination, implementation, and evaluation. Collaborate with insurance carriers, medical care providers, attorneys, employers, and employees; monitor progress and report results to employers and insurers. Maintain autonomy with minimal supervision and proficient patient education. Travel locally to perform in-person patient appointments ( Baltimore area, with travel to surrounding communities as needed). Qualifications Current, unencumbered Registered Nurse (RN) license in MD RN - WCCM required Registered with MCRSP within 30 days of start date Experience in case management of workers’ compensation cases; willingness to learn and take catastrophic cases Recommended certifications (CCM, CRRN, CDMS, CRC) preferred; REQUIRED within two years of hire Skills: organization and timeline adherence, PC and technology savvy, strong written, verbal, and interpersonal communication Ability to operate autonomously with minimal oversight Proficient in patient education Valid Driver’s License and good driving history Benefits Health and wellness – PPO, HDHP, and HMO options with Cigna and Kaiser (CA employees only).
