Job Overview:

We are currently seeking a compassionate and skilled Home Health Physical Therapist PT to provide personalized, one-on-one therapy in a home setting in Danville, VA. This position is ideal for a Physical Therapist who enjoys working independently while helping patients regain strength, mobility, and overall functionality in their daily environment.

Responsibilities of Home Health Physical Therapist PT:

• Provide home-based physical therapy to patients recovering from illness or injury
• Conduct initial assessments and develop individualized treatment plans
• Monitor patient progress and modify care plans as needed
• Assist patients with exercises and hands-on therapy to improve mobility and manage pain
• Educate patients on their plan of care, therapy goals, and self-care techniques
• Coach family members on how to safely assist with care and exercises
• Collaborate with interdisciplinary team members (RN, LPN, CNA, OT, SLP)
• Maintain accurate and timely documentation

Qualifications of Home Health Physical Therapist PT:

• Graduate of an accredited Physical Therapist program
• Active PT license in the state of Virginia
• Minimum of 2 years of physical therapy experience (home health preferred)
• CPR certification and TB baseline screening
• Strong clinical judgment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to work independently in a home setting
• Willingness to travel throughout the service area
