What You'll Be Doing Provide advanced wound care assessments, treatment planning, and ongoing management. Conduct routine rounding visits throughout assisted living, memory care, skilled nursing, and long-term care communities. Manage medications, diagnostics, treatment plans, and follow-up care. Collaborate with physicians, nurses, therapists, caregivers, and facility staff. Support chronic disease management, preventative care initiatives, and continuity of care. Help reduce avoidable hospitalizations and improve patient outcomes. Qualifications Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred). Experience in wound care, geriatrics, primary care, post-acute care, or related settings preferred. Strong clinical judgment and confidence managing medically complex patient populations.
