Job Description

We are seeking a dedicated Physical Therapist to join our therapy department. You will be responsible for the evaluation, treatment planning, and implementation of therapeutic interventions for patients across various settings including outpatient, acute care, and skilled nursing facilities (SNF). The facility serves patients of all ages and diagnoses, with specialties in wound care, lymphedema, pelvic floor, and pediatrics.

Responsibilities

• Conduct evaluations and develop individualized treatment plans.
• Implement therapy interventions based on patient needs and conditions.
• Collaborate with interdisciplinary teams to optimize patient care.
• Maintain accurate patient records and documentation using Epic charting system.
• Provide education and support to patients and their families.

Required Experience / Certifications / Licensure

• Minimum of 1 year of experience as a Physical Therapist.
• Current AK Physical Therapist license.
• BLS certification required.
• Experience in pediatrics and outpatient settings preferred, but not required.
