Schedule / Job Type

Contract | Full-Time | Days | Monday–Friday, 8:00 AM–5:00 PM | Onsite

Key Responsibilities

• Provide physical therapy services to patients within a home health setting across an assigned territory
• Manage a caseload with an expected productivity of 30 points per week (approximately 6 points per day)
• Conduct evaluations, develop individualized treatment plans, and deliver skilled PT interventions in the patient's home
• Document patient care accurately and in a timely manner using HCHB (Homecare Homebase) software
• Coordinate with the interdisciplinary care team to ensure continuity and quality of patient care

Required Qualifications

• Active Physical Therapist (PT) license in the state of Oregon
• Previous home health experience required
• Previous HCHB (Homecare Homebase) experience required
• Must reside within or be able to service a 50-mile radius from the office location

Preferred Experience / Skills

• Strong time management and self-directed work skills suited for an independent home health environment
• Experience managing productivity standards in a home health or field-based therapy role
