Position Title: Interventional Cardiologist/Endovascular Medicine Specialist

Division: OHC 

Reports To: Sr. Director of Specialty/CMO

JOB SUMMARY

The Interventional Cardiologist / Endovascular Medicine Specialist provides physician-level cardiovascular, vascular, venous, and structural heart consultation services for patients through Oyate Health Center. This role will primarily focus on coronary artery disease, patent foramen ovale and atrial septal defect evaluation, vascular disease, and venous disease. The position emphasizes timely outpatient specialty access, diagnostic interpretation, procedural planning, care coordination, clinical quality, and collaboration with primary care, referrals, PRC, diagnostic services, emergency services, and external procedural partners to deliver safe, culturally grounded care close to home.

ESSENTIAL FUNCTIONS:

The following functions are fundamental to the position and are required of all incumbents, with or without reasonable accommodation. These functions define the role and are not an exhaustive list of all duties.

Provides physician-level cardiovascular assessment, diagnosis, treatment planning, and follow-up for patients with coronary artery disease, PFO/ASD concerns, peripheral vascular disease, venous disease, hypertension, and related cardiovascular or vascular conditions within scope of licensure, credentialing, privileges, and available service capabilities. 

  • Provides physician-level cardiovascular assessment, diagnosis, treatment planning, and follow-up for patients with coronary artery disease, PFO/ASD concerns, peripheral vascular disease, venous disease, hypertension, and related cardiovascular or vascular conditions within scope of licensure, credentialing, privileges, and available service capabilities. 
  • Evaluates patients for interventional cardiology, vascular medicine, endovascular medicine, venous disease, and selected structural heart needs, including appropriateness for cardiac catheterization, coronary intervention, PFO/ASD evaluation, peripheral vascular intervention, endovenous intervention, vascular imaging, limb-salvage evaluation, and related specialty procedures.

  • Orders, reviews, interprets, and follows up on cardiovascular and vascular diagnostic testing, including electrocardiograms, echocardiograms, nuclear cardiology studies, stress testing, ambulatory monitoring, vascular ultrasound studies, arterial and venous studies, laboratory results, and advanced imaging when indicated.

  • Uses vascular interpretation expertise to support appropriate review, follow-up, referral, and care planning for vascular ultrasound and related noninvasive vascular diagnostic studies.

  • Develops evidence-informed care plans for coronary artery disease, PFO/ASD concerns, vascular disease, venous disease, and related cardiovascular conditions, including medication management, antiplatelet or anticoagulation considerations, risk-factor modification, lifestyle counseling, monitoring plans, procedural planning, and timely specialty follow-up.

  • Performs or coordinates interventional cardiology, endovascular, vascular, venous, and structural heart procedures only within approved privileges, available facility resources, clinical protocols, payer requirements, and applicable federal, state, tribal, accreditation, and organizational standards.

  • Coordinates pre-procedure evaluation, informed consent, risk assessment, medication reconciliation, antiplatelet or anticoagulation planning, procedure readiness, external procedural scheduling, discharge planning, and post-procedure follow-up for cardiac, vascular, venous, pulmonary embolism, and structural heart interventions.

  • Identifies urgent or emergent cardiovascular and vascular concerns, including acute coronary syndrome, unstable arrhythmias, decompensated heart failure, cardiogenic shock concerns, pulmonary embolism, critical limb ischemia, stroke symptoms, acute vascular compromise, and other high-risk presentations, and initiates appropriate escalation, emergency response, transfer, or consultation processes.

  • Supports early identification and management of peripheral arterial disease, acute limb ischemia, and chronic limb-threatening ischemia through screening, diagnostic review, risk stratification, referral coordination, procedural planning, and follow-up care aimed at reducing avoidable complications and improving limb-preservation outcomes.

  • Provides physician expertise for complex coronary, PFO/ASD, peripheral vascular, venous, and endovascular cases, including coordination with external hospitals, catheterization laboratories, procedural teams, and specialty partners when services are not available on site.

  • Collaborates with primary care, tertiary center, specialty providers, nursing, referrals, PRC, pharmacy, laboratory, imaging, care management, and external hospital or procedural partners to support safe, timely, coordinated cardiovascular, vascular, venous, and thromboembolic care.

  • Supports development of cardiology, interventional cardiology, vascular medicine, endovascular medicine, venous disease, and selected structural heart workflows, including referral pathways, diagnostic follow-up processes, procedural coordination, quality tracking, patient education, access-to-care improvements, and external referral pathways for heart failure and electrophysiology services.

  • Participates in clinical quality, outcomes review, patient safety, utilization review, research-informed practice, and specialty service development initiatives related to cardiovascular, vascular, venous, and endovascular care.

  • Provides education to patients and families regarding cardiac, vascular, venous, and thromboembolic conditions; warning signs; medication adherence; procedure preparation; recovery expectations; lifestyle modification; risk reduction; and follow-up needs.

  • Maintains accurate, timely, and complete documentation in the EHR in accordance with clinical, payer, regulatory, quality, coding, billing, credentialing, and organizational standards.

  • Must comply with federal laws and regulations as required by the Health Insurance Portability and Accountability Act (HIPAA).

  • Maintains regular and reliable attendance consistent with the position’s schedule and operational needs.

  • Communicates effectively with internal and external stakeholders as required by the role.

  • Adheres to all applicable policies, procedures, clinical protocols, credentialing requirements, privileging standards, payer requirements, and regulatory requirements.

  • Utilizes required systems, tools, and technology necessary to perform the job.

  • Other duties as assigned.

Clinical provider responsibilities must be performed within licensure, credentialing, privileging, collaborative practice requirements, and applicable federal, state, tribal, and organizational standards.

  • Valid driver’s license must be kept current, and certificates, credentials or licenses must be kept current and consistent with regulations required by applicable federal, state and/or grant regulations.

  • Clinic-based setting with direct patient care, extensive EHR use, patient communication, diagnostic follow-up, and coordination with internal and external specialty care partners. Reasonable accommodations may be made.

REQUIRED EDUCATION/EXPERIENCE:

  • Doctor of Medicine (MD), Doctor of Osteopathic Medicine (DO), MBBS, or equivalent medical degree from an accredited medical school required.
  • Completion of accredited residency training in Internal Medicine required.
  • Completion of accredited fellowship training in Cardiovascular Medicine required.
  • Completion of fellowship training in Interventional Cardiology required.
  • Advanced fellowship training or demonstrated expertise in Endovascular Medicine, Vascular Medicine, Structural Heart Disease, or closely related cardiovascular subspecialty area preferred based on service scope.
  • Board certification or board eligibility in Internal Medicine and Cardiovascular Disease required.
  • Board certification or board eligibility in Interventional Cardiology required.
  • Board certification, certification, or documented expertise in Vascular Medicine, Endovascular Medicine, Nuclear Cardiology, Echocardiography, and/or vascular interpretation preferred.
  • Current, unrestricted physician license or eligibility for licensure in South Dakota required.
  • Current DEA registration and prescriptive authority required or must be obtained within organizational timelines.
  • Current BLS certification required.
  • Experience evaluating and managing coronary artery disease, PFO/ASD concerns, vascular disease, venous disease, hypertension, and related cardiovascular or vascular conditions required.
  • Experience with cardiac catheterization, interventional cardiology, endovascular procedures, vascular imaging, limb-salvage evaluation, pulmonary embolism intervention, procedural coordination, and multidisciplinary cardiovascular care preferred.
  • Experience in cath lab leadership, cardiovascular research, clinical education, quality improvement, or specialty program development preferred.
  • Experience with EHR documentation, diagnostic follow-up, care coordination, referral management, quality improvement, and interdisciplinary team-based practice required.
  • Experience in Tribal health, IHS, rural health, underserved communities, or 638 facilities preferred.
  • Must successfully pass a criminal background check, credentialing review, privileging process, provider enrollment requirements, and pre-employment drug screen.
