Community Health Worker

The Field Based Community Health Worker is responsible for identifying and initiating referrals for social service programs, including financial, psychosocial, community and state supportive services. As a Field Based Community Health Worker (CHW), will interact with Medicare and Medicaid members to ensure to provide home and social resources and member resource education. The Field Based Community Health Worker also addresses social determinants of health such as transportation, housing, and food access. Primary responsibility for the community health worker is to locate and interact with assigned members to bridge medical care appointments and collaborate with both medical management teams and primary care for best clinical and social outcomes. 1)Identify simply and complex needs of the member through observation and patient disclosure; 2) Gain cooperation with the member to complete community and employed referral sources for more actions 3) Meet people in their living environments as safety permits 4) Produce daily, weekly and monthly documentation as assigned by the Director 5) gain assistance of professional team members to facilitate care. Primary Responsibilities Create a positive experience and relationship with the members Help members set goals and develop a care plan to achieve those goals Proactively engage the members to manage their own health and healthcare As needed, help the member engage with mental health and substance use treatment Utilize both company and community-based resources to establish a safe and effective case management plan for members Collaborate with patients, family, and healthcare providers Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services Document referrals provided Accountable to understand role and how it affects utilization management benchmarks and quality outcomes Be flexible, adaptable and, above all, patient in all types of situations Engage members either face to face or telephonically Help members set person-centered goals and develop checklist of resources to achieve those goals with regular follow- up calls and ongoing documentation of progress towards goals met Adhere to detailed, specific documentation requirements in the member's health record Proactively engage the members to manage their own health and healthcare using Motivational Interviewing Skills Provide member education on community resources and benefits Utilize solid skill sets of managing multiple tasks at a time, being self-motivated, driven toward quality results, managing time well, being very detailed oriented and organized, working well in a team and on your own, and managing multiple deadlines Provide member education on community resources and benefits Partner with care team (community, providers, internal staff) Knowledge and continued learning of community cultures and values Performs other duties as assigned You’ll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications High School Diploma/GED 1+ years of computer proficiency experience including MS Word, Excel and Outlook 6+ months of field-based experience of community advocacy Solid knowledge of regulatory requirements, competent in case management business as well as clinical arenas Demonstrated solid technology skills and excellent interpersonal skills Demonstrated ability to negiotiate and arbitrate without difficulty Demonstrates activities for follow-through Ability to travel locally within Las Vegas Metro and complete documents for Reimbursed mileage Resides within the local community Unrestricted Driver's License Preferred Qualification Experience in case management, community outreach or volunteer activities with interaction with the public Competencies For High Performers Participation in departmental projects assigned by Director or Manager Maintains productivity standards for at home visits and phone management set by the director of the program Participates in safety programs as deemed required for home and driving locations Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $20 - $36 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
