Position Summary

The Program Specialist is responsible for coordinating, monitoring, and supporting services for individuals with intellectual disabilities and autism in accordance with Pennsylvania Chapter 6400 regulations and Office of Developmental Programs (ODP) requirements. This position serves as a liaison between individuals, families, Supports Coordinators, medical professionals, community resources, and Lehigh Human Support Services to ensure services are person-centered and responsive to each individual's needs, preferences, and goals.

The Program Specialist is responsible for coordinating assessments, participating in the Individual Support Plan (ISP) process, supporting implementation of services, promoting community integration, and encouraging communication and involvement with families and friends. This position plays a critical role in maintaining regulatory compliance while ensuring quality supports and meaningful outcomes for the individuals served.

Essential Duties and Responsibilities

• Coordinate the completion of assessments and ensure required documentation is completed and maintained.

• Complete and maintain required documentation, including quarterly reports, annual assessments, incident-related documentation, and other records required by ODP regulations and agency policies.

• Participate in the development, implementation, and review of Individual Support Plans (ISPs).

• Coordinate annual ISP meetings, revisions, team reviews, and monthly monitoring activities with Supports Coordinators and other team members.

• Collaborate with interdisciplinary teams to ensure services are delivered in accordance with individual plans and regulatory requirements.

• Review ISP information with individuals and direct support staff to ensure understanding and proper implementation of goals and supports.

• Monitor individual progress and communicate concerns, updates, and recommendations to appropriate team members.

• Act as a liaison between individuals, families, Supports Coordinators, medical providers, employers, community agencies, and LHSS staff.

• Provide training, guidance, and support to staff regarding individual needs, goals, ISP implementation, and regulatory requirements.

• Offer support and informal counseling to individuals as needed.

• Provide backup support for program operations and other staff as needed.

• Participate in emergency response situations in accordance with agency policies and established procedures.

• Plan, coordinate, and supervise activities for individuals in accordance with their Individual Support Plans (ISPs).

• Support individuals in achieving personal goals and participating in meaningful community activities and relationships.

• Promote community integration and inclusion in accordance with individual preferences and goals.

• Facilitate communication and involvement between individuals, families, friends, Supports Coordinators, and other members of the support team.

• Participate in and assist with coordinating medical appointments and healthcare services as needed.

• Assist with maintaining compliance with ODP regulations, agency policies, and quality assurance standards.

• Perform other duties as assigned by the Quality Manager.

Minimum Qualifications

Candidates must meet one of the following ODP Chapter 6400 Program Specialist qualifications:

• Master's degree or higher from an accredited college or university and at least one (1) year of experience working directly with individuals with intellectual disabilities and/or autism; or

• Bachelor's degree from an accredited college or university and at least two (2) years of experience working directly with individuals with intellectual disabilities and/or autism; or

• Associate's degree or at least 60 college credits from an accredited college or university and at least four (4) years of experience working directly with individuals with intellectual disabilities and/or autism.

Additional Requirements

• Valid driver's license and acceptable driving record.

• Ability to travel to residential locations, meetings, appointments, and community activities.

• Strong written, verbal, and interpersonal communication skills.

• Strong organizational, time management, and documentation skills.

• Ability to work independently and collaboratively with interdisciplinary teams.

• Knowledge of ODP regulations, person-centered planning, and ISP development preferred.

• Experience working in residential, community, or human services settings preferred.

Working Environment

The Program Specialist works within residential and community-based settings and maintains regular contact with individuals receiving services, families, Supports Coordinators, medical professionals, and agency staff. The position requires flexibility, attention to detail, effective communication, and a commitment to supporting individuals in achieving meaningful outcomes and living fulfilling lives within their communities.
