Our expert physicians create cutting-edge treatment plans that best serve the needs of each individual and improve their quality of life./ph3Responsibilities:/h3ulliPerforms on-site screening clinical assessments to determine appropriate patient treatment plans of residents at client facilities by the completion of a comprehensive assessment/liliFunctions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill perioperative patients/liliOrders and interprets diagnostic and therapeutic tests relative to patients age-specific needs/liliAssists with the provision of care in accordance with facility, state, and federal regulations/liliMonitors the effectiveness of interventions/liliMaintains relationships with all appropriate contacts in facilities/liliEducates family members, clients, and prospective accounts regarding services and programs/liliAssists with the development and maintenance of physician relations, as assigned/liliCompletes weekly, monthly, and other reports, as assigned, in a timely and comprehensive manner/liliOther duties as assigned by Medical Director/li/ulh3Qualifications:/h3ulliCurrent Nurse Practitioner/Physician Assistant License/liliDegree from an accredited Nurse Practitioner/Physician Assistant Program/liliACLS/liliCurrent infection control certification/liliStrong clinical collaboration with physicians/liliProficient computer skills/liliPACS and EMR experience preferred/liliCommitted to professional growth and learning/liliClean driving record/liliPersonally owned vehicle required/liliLocal travel required/liliNot listed on the OIG exclusion list/li/ulh3Preferred Experience:/h3ulliVascular Care: Venous Insufficiency, Peripheral Arterial Disease/liliImage Guided Injectables/liliAdvanced Geriatric/liliSkilled Nursing Facilities, Assisted Living Facilities/li/ulh3Benefits:/h3ulli401(k)/liliDental insurance/liliHealth insurance/liliPaid time off/liliVision insurance/liliFSA Plan/li/ul/div
