Overview

Responsibilities
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and the Interdisciplinary Team.
- Contribute to the development and execution of an age-appropriate, culturally relevant plan of care; involve the resident and family in the process; adjust the plan based on outcomes.
- Recommend revisions to the plan of care and coordinate referrals to ensure all aspects of the resident’s needs are addressed.
- Provide direct and indirect skilled nursing care under the supervision of a Registered Nurse; maintain a safe environment and adhere to policies and procedures.
- Support mentoring and orientation of staff; exercise leadership in the professional practice setting under RN direction.
- Maintain current knowledge and competency through ongoing education and in-service activities.

Requirements
- Active New York LPN license
- BLS certification (American Heart Association or RQI preferred)
- Minimum 1 year of LPN experience in a long-term care setting
- Graduation from an approved LPN program (LPN diploma or equivalent)
- Ability to work in a long-term care setting with rotating shifts
- Must be able to start on a Tuesday or Thursday for orientation and participate in full-shift orientation sessions
