Position Summary:

The RN is expected to provide comprehensive 1440 care to patients through the activities of assessment, planning, and provision of direct nursing care within our policies and standards.

Required Experience:

• Licensed as an RN through the State Board of Nursing.
• A minimum of 6 months nursing experience within the last two years.
• Graduate of an accredited school of nursing.
• Compliance with accepted professional standards and practices.
• Self-directed with the ability to work with little supervision.
• Must possess and maintain good physical stamina and mental health.
• Must have excellent observation, good nursing judgment, and good oral and written communication skills.
• Avid team player.

Responsibilities:

• Complete assessments (pre-move in, move-in, routine, change in condition, focused, post-fall, cognitive)
• Update care plans
• Medications: check in cycle fill, process and complete order changes, communicate with pharmacy, monitor effectiveness and side effects, med cart audits, refills, med destruction
• Coordinate care with third party providers including but not limited to hospice, primary care providers, clinics, case managers
• Communicate and educate residents and families
• Process and implement orders
• Provide education to care team on cares and medications
• Assist with afterhours nursing calls
• Simple wound care -observations of skin tears, wounds between SN visits
• Referrals for therapy, podiatry, SN
• Assisting in Memory Care with behaviors, problem solving
• Take directions/delegations from Wellness Director to support wellness office, residents in the community and prospect assessments
• Additional duties as assigned

General Working Conditions:

This position entails standing for long periods of time. While performing the duties of this job, the employee is required to communicate effectively with others, sit, stand, walk and use hands to handle keyboard, telephone, paper, files, and other equipment and objects. The employee is occasionally required to reach with hands and arms. This position requires the ability to review detailed documents and read computer screens. The employee will occasionally lift and/or move up to 25 pounds. The work environment requires appropriate interaction with others. The noise level in the work environment is moderate. Occasional travel to different locations may be required.
