Homecare Care Manager Bilingual (Must Speak English and Spanish) Location: Lakewood, Colorado Company: Family and Friends Homecare Salary: $29 HOURLY Benefits: n/a Job Type: FULL_TIME Minimum Years of Experience: 1 Level of Education: n/a About the Role: Family & Friends Homecare is seeking a detail-oriented, compassionate Care Coordinator who is fluent in both English and Spanish to support our clients, caregivers, and Case Managers/Referral Sources. This role is critical to ensuring services are delivered smoothly, communication is clear, and families feel supported throughout their care journey. Key Responsibilities: Serve as a primary point of contact for clients, families, caregivers, and referral partners Meet with potential clients and their families Perform intake visit with New Clients Create Care Plans for HCBS and IHSS Medicaid Clients Communicate with Case Managers regarding Client Care Plans Understand Medicaid Eligibility and PARs Schedule and Complete Quarterly Supervisory Visits with Clients Assist with the Incident/Complaint Reporting process, following up on incidents or complaints and ensuring proper and timely communication Maintain accurate client records Qualifications: Fluent in English and Spanish (required) Previous experience in care coordination, home care Strong organizational and time-management skills Excellent communication and customer service skills Comfortable using software, email, and basic office systems Ability to multitask in a fast-paced environment Knowledge of Medicaid, home care What We Offer: Competitive pay Weekly pay Supportive leadership and a positive team-oriented culture Meaningful work making a real difference in families' lives Full-time position Why Family & Friends Homecare?
