- HHA - Iris (1235)Iris Home Health Care Agency - West Palm Beach, FL 33407## DescriptionJob Responsibilities:Duties of the Home Health Aide (HHA) include, but are not limited to: 1. Provides assistance with personal care, hygiene and activities of daily living.2. Encourages client participation in activities to the extent to which the client is able3. Performs duties of a Home Health Aide (HHA) as per: Scope of Practice and Florida Law4. Assists client to: a.)bed, b.)commode, c.)chair, and assist with ambulation5. Turns and positions bed bound clients6. Measures and records intake/output, as assigned7. Measures and records temperature, pulse and respiration on each visit8. changes bed linen if needed9. Prepares simple meals following dietary instructions as instructed10. Maintains a neat and clean environment11. May grocery shop one time a week for a list of ten items or less as needed12. Informs supervisor of any changes in client's condition or home situation13. Follows care plan as written14. Provides documentation of care given on Agency approved forms15. Reports any changes in patient's condition, living conditions etc., to DON/Supervisor as they occur16. Performs any other task/duty that is specifically assigned by supervisor, and for which Home Health Aide (HHA) has been specifically trained. Documentation of specific training must be included in employee's personnel file17. Conducts self in professional manner at all times and in all situations18. Provides Agency with: required certifications, required license, necessary information to be able to verify experience, documentation of CEUs19. Accepts only those assignments for which he/she is qualified as per Florida Law20. Complies with all Agency policies and procedures21. Communicates with Agency about any problems or concerns22. Complies with HIPPA Privacy Rules, Policies and Procedures23. Reports any suspected violations of Privacy Practice to Privacy Official as soon as breach/possible breach is identified.## QualificationsJob Qualifications1. Must provide evidence of formal training2. Must provide physical Home Health Aide (HHA) certificate3. Must provide documentation of clearance to work (green card, employment authorization card, social security, etc...)4. Must provide evidence of competency training and evaluation as well as evidence of at least annual attendance at in-service education programs5. Must be able to use Electronic Visit Verification (EVV) program on own cellular/ mobile device6. Must be able to provide own transportation7. Minimum of one (1) year current experience is required
