We’re currently hiring for a Visiting Nurse to provide mobile primary care in the Blue Ridge area.This is a full-time opportunity with flexibility. Our mobile approach allows you to provide care directly in patients’ homes and communities rather than being tied to a traditional office or hospital setting.Do you have a passion for helping people? At Your Health we provide care to patients and provide support to family members.Responsibilities:Looking for someone with a Hospice heart and passion for providing end of life careProvide Case Management to patients and families, this includes symptom management, medication management, and collaboration with members of the IDGResponsible for overseeing patient plan of care with the collaborating physicianProvide supervisory visits and oversight for LPN and CNAsProvide education to patients, families, and facility staffProvide oversight to Aides#IND123Qualifications:Must have a RN license unincumbered in the state you are applyingReliable transportation and valid driver's license, clean MVRExperience in primary care, home health, or hospice is preferred. #J-18808-Ljbffr
