Remote Care Coach for Chronic Conditions
Humana Inc
Humana Inc. is seeking a Care Coach to support members remotely, guiding them toward optimal wellness and coordinating with clinical and community resources. The role emphasizes chronic condition management, data review, and compliance within a regulated program.
Qualifications include an Associate degree, 2+ years in direct adult patient care, and strong communication and EMR skills. Prior experience with Medicare/Medicaid is a plus. Training is provided for the remote setup.
