The Wellness Center provides no-cost preventative care, low-cost acute care appointments, behavioral health services, and prescription dispensing for select prescriptions at no additional cost!Qualifications:At least one of the following qualifications is required:Bachelor's degree from an accredited college or university with a relevant majorAssociate's degree from an accredited college or university with a relevant major and two (2) years of relevant experienceHigh school diploma or equivalent and five (5) years of relevant experience with progressive increase in responsibilitiesHolds valid Florida certification in the area of Physical TherapyHolds professional license as a Physical TherapistPerformance Responsibilities:Practices according to Chapter 486, Florida Statutes.Provides services including, but not limited to: screening, evaluation, educationally relevant treatment, program planning and consultation.Evaluates need for services in accordance with state and district guidelines and submits reports.Consults with educational staff, parents, and other health professionals regarding students staffed into Physical Therapy Program.Participates in staffing/IEP conferences as assigned.Develops goals and objectives for Individualized Education Plans and/or plan of care using data collected during therapy sessions, evaluations and input from parents and school staff.Schedules and implements comprehensive therapy treatment based upon the evaluation procedures and referral/prescription of the student's physician.Prepares progress reports for all students receiving physical therapy services on a regular basis.Trains, schedules and supervises Physical Therapist Assistants, paraprofessional, or other staff.Utilizes electronic systems of data collection and reporting (including Medicaid billing as appropriate) in accordance with district guidelines.Performs other job-related functions as assigned.Charlotte County Public Schools promote Equal Educational and Employment Opportunities.
