Location 7435 W Talcott Ave, Chicago, IL 60631 Overview Department: Case Management Schedule: PRN (As needed) Days Facility: Resurrection Medical Center Location: 7435 W Talcott Ave, Chicago, IL 60631 Case management and milieu management responsibilities include emotional support, crisis intervention, counseling, information and referral, case management, and discharge planning for patients and families dealing with hospitalization, illness, disability, or trauma. Responsibilities Provide emotional support, crisis intervention and counseling; offer information and referral services; refer for financial assistance and supportive services; conduct psychosocial assessments; assist in the development and re‑evaluation of Treatment Plans; collaborate with a multidisciplinary team; coordinate discharge planning; complete necessary documentation and maintain clinical records for each client during their length of stay. Qualifications Education, Experience, Training Bachelor’s degree in Social Work, Marriage and Family Therapy, or other related field. Current registration as an intern with a Board, as appropriate to education and licensing requirements. Minimum one (1) year inpatient or outpatient behavioral health experience preferred. Preferred Qualifications Current BCLS certificate upon hire and maintained current. Masters in Psychology, Social Work, Marriage and Family Therapy, Licensed Professional Clinical Counselor, or related field. Pay Transparency Resurrection Medical Center offers competitive compensation; a flat rate of $36.39 per hour. The exact starting compensation will be determined at the time of selecting an applicant for hire, taking into account skillset, years of applicable experience, education, credentials, and licensure.
