Job Description: Day shift, 3 -12 hour shifts, 7am-7:30pm; 8:30a-9pm; On call rotation and every other weekend. Minimum of 18 months recent Dialysis experience. Must have 18 months perm Dialysis MUST HAVE PERM OHIO LICENSE OR COMPACT STATE IN HAND. AHA or American Red Cross BLS & ACLS required. ONE recent references from a manager, ANM, supervisor or director in the specialty applying for are REQUIRED to qualify for submission.

We provide IHD - hemodialysis services in both a dialysis unit for patients who are stable enough to come there, and at the bedside of ICU patients who need hemodialysis and cannot travel. We also connect and disconnect patients from their peritoneal dialysis treatment. We do not provide CRRT therapy- that is managed by ICU nurses. For the actual machines for IHD we primarily have Fresenius machines but we would provide a full orientation to the machines themselves. We do not do PIRRT / use Tablo Machines at all at FV.

Standard Qualivis Cover Sheet completed and filled in; please submit candidate's interview availability within cover sheet in the notes section. All RTO to be discussed during interview and must be included at time of submission. Travelers may be submitted for 1 job at a time within the Cleveland Clinic Health System Experience: 18 months combined perm experience in specialty applying for required, preferred to be within one location but not required. Must have a PERMANENT Ohio license in hand at time of submission. Compact licenses are accepted for any starts 1/1/23 and forward. *TWO references from a manager, ANM, supervisor or director in the specialty applying for are REQUIRED to qualify for submission. Please no co-worker references, these will not be considered, and profile will be rejected. References must be from assignments worked in the past 12 months. Required certifications need to be ACTIVE with nothing pending. Locals accepted at same BRPBDS Assessment is waived. ECG rhythm recognition test and safeMedicate will be completed during orientation, an acceptable score on both assessments is required. Traveler will receive 2 attempts maximum to pass, otherwise will be canceled. BG hits of any kind are reviewed by the facility but will not accept any traveler with an open case. Facility does not accept any license hits. EFFECTIVE 9/30/22: Nurses and CSTs are EXEMPT from Nicotine Free Facility Requirements. Exemption is temporary and will be honored until further notice. All other travelers (aside from Nurses and CSTs) MUST BE NICOTINE FREE - Negative 10 Panel + Nicotine (Cotinine) Drug screen required within 12 months of start. Must be updated annually. Permanent and per diem candidates cannot return to Cleveland Clinic hospitals for 1 year from their previous end date. IMPORTANT - BCI/FBI prints are time Sensitive - Must completed within 48 Hours of signing contract. Extensions: Candidates may extend with client for a period of 18 months; after which a 6 month break is required. Client may choose to defer the required 6 month break on a case by case basis. Please reach out to the Account Manager if Client approval is needed. Modules: 4 hours of modules are billable, any additional time required for pre-start modules is non-billable. Average 6-15 hours of modules Modules are completed pre-start and annually, and time needed varies by specialty Annual modules are billable up to 4 hours, this only pertains to annual modules required by the facility and applies to travelers have been on contract for a continuous year.
