Community Health Worker

Community Health Worker (CHW) Responsibilities & Duties Conduct home visits to educate community members on health practices and resources Assist individuals in managing chronic conditions and accessing healthcare services Provide culturally appropriate health education and information Support community members in navigating the healthcare system Advocate for individual and community health needs Collect data and relay information to healthcare providers to inform care Develop and implement community outreach programs Facilitate access to social services and other community resources Organize and participate in health screenings and immunization clinics Collaborate with healthcare providers and community organizations Build trusting relationships with community members Empower individuals to adopt healthy behaviors and lifestyle changes Community Health Worker (CHW) Qualifications & Skills Previous experience in health education or community outreach Strong knowledge of community resources and services Ability to work with diverse populations Excellent communication and interpersonal skills Training in motivational interviewing Experience in data collection and reporting Background in public health or social work High school diploma or GED Strong organizational and problem‑solving skills Ability to work independently and as part of a team Basic computer skills, including familiarity with Microsoft Office Valid driver's license and reliable transportation Comfortable working in various community settings Demonstrated commitment to improving community health Effective communication skills Cultural competency and sensitivity Position is subject to successful completion of a drug screen and background check.
