Office Location: We strongly prefer candidates who are able to work out of our New York City office location (505 Eighth Avenue, New York, NY 10018). We have a hybrid schedule of working 2 days in the office and 3 days remote. We are also open to candidates based elsewhere in the United States. 

GENERAL STATEMENT OF RESPONSIBILITIES

The Practice Facilitator works as part of CAI’s National Technical Assistance Program (NTAP) for Clinical and Preventive Excellence multidisciplinary team, funded by the Health Resources and Services Administration (HRSA), to plan and provide direct, hands-on technical assistance (TA) to the nation's community health center network. This includes partnering with health centers to identify gaps in performance, causal factors, and to identify and implement changes to improve clinical and preventive care health outcomes. 

The Practice Facilitator's work is grounded in systems strengthening, implementation science, and continuous quality improvement frameworks using a relational approach to TA, which emphasizes building and nurturing trust-based partnerships with health centers. This approach prioritizes understanding each health center's context and needs, engaging diverse staff perspectives, and maintaining the reliability and consistency needed to sustain long-term, collaborative relationships that lead to meaningful and lasting change.

Specifically, Practice Facilitators use data and information to guide and support assigned health centers participating in 12-month Communities of Practice (CoPs) and 3-Year Outcomes-Focused Cohorts focused on improving health outcomes for patients with chronic disease (e.g., hypertension, diabetes) and strengthen the provision of preventive and supportive services.

MAJOR TASKS AND RESPONSIBILITIES (include, but are not limited to)

• Partner with the NTAP project team and subject matter experts in the design and facilitation of CoP and Outcomes-focused virtual and in-person learning sessions.
• Build and sustain trust-based, collaborative partnerships with assigned health centers, consistent with CAI's relational approach to technical assistance – including building buy-in from health center leadership.
• Partner with health center staff across roles and levels of authority to assess health center context and capacity and engage differing perspectives in planning and decision-making to ensure technical assistance is tailored for each health center’s unique context. 
• Facilitate data-informed process to identify gaps in client outcomes, causal factors, priority areas for improvement, and strategies to address causal factors to improve client health outcomes.
• Facilitate development of mutually agreed upon health center improvement plan including documented implementation support strategies (e.g., new workflows, strengthened community partnerships, team-based care models, clinical consultation, protocol development, staff training) tailored to each health center's unique context.
• Support use of real-time data to inform health centers’ continuous improvement activities.
• Provide data-driven iterative implementation support through regularly scheduled virtual health center team huddles to support implementation of mutually agreed upon plan and to rapidly identify and address implementation challenges.
• Support health centers in testing and refining changes to care delivery through Plan-Do-Study-Act (PDSA) cycles and other rapid-cycle improvement methods.
• Maintain reliability and consistency in fulfilling technical assistance commitments to reinforce health centers' trust and confidence in the partnership.
• Document technical assistance activities, findings, and health center progress in QuickBase, CAI’s project management platform, and track a defined set of performance measures for each assigned health center.
• Collaborate with CAI CoP and Cohort Leads, subject matter experts, evaluators, and partner organizations to ensure technical assistance is current, evidence-based, and responsive to health center needs.
• Identify and communicate emerging issues, barriers, and promising practices to program leadership to inform continuous improvement of NTAP's technical assistance approach.

Other duties as necessary/assigned 

MINIMUM EDUCATION, EXPERIENCE AND OTHER REQUIRED SKILLS:

• Bachelor's degree in public health, nursing, health administration, or related field; Master's degree preferred.
• Minimum of 3 years of experience in primary care, quality improvement, health center operations, or a related clinical/administrative setting.
• Applied experience providing technical assistance, practice facilitation, or quality improvement support to healthcare organizations.
• Experience working with Federally Qualified Health Centers (FQHCs) or other safety-net primary care settings preferred.
• Demonstrated ability to build and sustain trust-based, collaborative relationships with diverse stakeholders, consistent with a relational approach to technical assistance.
• Working knowledge of continuous quality improvement (CQI) methods, including PDSA cycles and root cause analysis.
• Familiarity with implementation science frameworks (e.g., EPIS) and adult learning principles.
• Comfort using and interpreting UDS and EHR data to identify performance gaps and track progress.
• Excellent interpersonal, oral, and written communication skills, with the ability to build rapport with diverse clinical and administrative teams.
• Strong organizational skills and ability to manage a caseload of multiple health centers simultaneously.
• Advanced knowledge of Microsoft Word/Excel/PowerPoint/Outlook ; experience with project management platforms (e.g., QuickBase) a plus.
• Willingness to travel approximately 20% annually, nationally.

The employee is often required to sit and work at a desk for prolonged periods of time and use their hands and fingers to handle or feel. The employee is occasionally required to stand, walk, reach with arms and hands, climb or balance, and stoop, kneel, crouch, or crawl. Must be able to lift up to 15 pounds at times. Vision abilities required by this job include close vision.

COMPENSATION:

The salary range for this position is $80,000 - $95,000, depending on experience.
