The ideal candidate combines strong clinical judgment with exceptional organization, communication, and attention to detail.Key ResponsibilitiesComplete comprehensive nursing assessments for newly admitted residents, including vital signs and immediate clinical needs.Review and document medications, allergies, diagnoses, medical history, previous procedures, and advance directives.Review admission orders and ensure necessary clinical information is communicated to the appropriate members of the care team.Coordinate with physicians, nursing leadership, case managers, floor nurses, therapy, and other disciplines to support a smooth transition of care.Complete required admission documentation accurately and within established timeframes.Identify and escalate changes in condition or immediate clinical concerns.Orient residents and families to the facility and help answer questions regarding the transition into care.Support development and implementation of the resident's initial plan of care.Maintain compliance with facility policies and applicable state and federal requirements.Promote a welcoming, compassionate admission experience for residents and families.Why Join Our Team?Providence Pointe isn't simply a place to work—it's a team with a long-standing reputation for quality care in the Paducah community.
