What You'll Do

• Provide and document nursing care to patients in a home setting, promoting health, preventing illness, and aiding recovery
• Collaborate with healthcare teams and community resources to coordinate patient care plans
• Evaluate patient responses to care interventions, adjusting as necessary
• Follow established protocols and guidelines to ensure safe, effective care delivery
• Maintain accurate clinical records and communicate effectively with patients and team members
• Exercise independent judgment and critical thinking in emergency situations
• Be flexible with work hours, including participating in on-call responsibilities

What You Need

• Valid Massachusetts LVN/LPN license
• Ability to exercise independent judgment and critical thinking
• Strong interpersonal and communication skills
• Ability to maintain confidentiality and develop clear clinical documentation
• Valid driver's license and ability to travel in inclement weather

• Prior experience in home health nursing
