Physical Therapist (PT) - Home Health

Job Summary

A dedicated and compassionate Physical Therapist is needed to provide home health services across multiple counties. The ideal candidate will manage a patient caseload primarily consisting of adults to geriatrics, focusing on various rehabilitation needs. The role emphasizes efficiency and productivity, with scheduled visits organized to minimize travel time.

Key Responsibilities

• Provide high-quality physical therapy services in a home health setting.
• Work with patients recovering from joint replacements, and managing chronic conditions such as COPD, CHF, and T2DM.
• Conduct assessments, develop treatment plans, and implement therapy interventions.
• Maintain accurate patient records and documentation using Home Care Home Base (HCHB) and Epic systems.
• Collaborate with healthcare professionals and families to support patient care goals.
• Adhere to productivity standards, including an average daily patient census.

Qualifications

• Minimum of 1 year of experience in physical therapy; beginners are welcome.
• Active Basic Life Support (BLS) certification.
• Licensed as a Physical Therapist in South Carolina or eligible for compact licensing.
• Familiarity with Home Care Home Base (HCHB) preferred; completion of an online HCHB course required if not experienced.

Work Setting Requirements

This position requires travel between patient homes within the designated counties, with an average daily travel distance of 20-30 miles. The clinician should be flexible and willing to float between facilities as needed.

Schedule

• Full-time position with a standard day shift of 8 hours.
• No weekend shifts required.
• Mandatory work during major holidays occurring within the assignment period, with expectations based on the number of holidays.

Compliance

Full compliance with organizational policies is required to start on time. Non-compliance may result in a delay of up to 2 weeks due to the orientation schedule.

• Travelers accepted at the same rate as local candidates.
• Scrub colors are flexible and any color is acceptable.
