Activities Director

Prepare and submit reports and charts to treatment to reflect resident/patient reactions and evidence of progress or regression. Compile and maintain current activity schedules and calendars. Assess the quality of individual plans and group programs by conducting interviews, visits and consultations with families and the treatment team. Ascertain and interpret group interests, evaluate equipment and facilities, and adapt activities to meet participant needs. Train principles, techniques and safety procedures to participants in recreational activities and demonstrate use of materials and equipment. Conduct therapy sessions to improve the mental and physical well-being of patients and residents. Plan organizes, directs and participates in treatment programs designed to provide meaningful involvement with residents and to facilitate patients' rehabilitation for the integration into the community and to mitigate further medical problems. Obtain information from medical records, medical staff, family members and residents/patients to assess capabilities, needs and interests. Utilize appropriate tools including a minimum data set instrument (i.e. MDS). Complete assessment protocols including RAI/RAPS. Instruct resident/patient activities and techniques, such as sports, dance, music, are or relaxation techniques, designed to meet specific physical or psychological needs. Confer with members of treatment team to plan and evaluate therapy programs. Encourage patients/residents with special needs and circumstances to acquire new skills and get involved in health-promoting activities such as games, arts, crafts, and gardening. Other duties as required to assure resident/patient involvement with their well-being. Supports and abides by the mission, values, and policies of CHCS in all activities.
