This role is responsible for ensuring exceptional patient care, efficient clinical operations, regulatory compliance, staff development, and collaboration across disciplines. The Clinical Team Coordinator plays a critical role in advancing our mission of delivering compassionate, patient-centered end-of-life care.Benefits Include:Mileage reimbursement at the federal rateGenerous time off including PTO and Sick TimeMedical, Dental, Vision and other benefits offered starting Day One!403(b) plan with matchEmployer paid Life and Short-term DisabilityBereavement leaveSelf-care coaching + EAPCompany tech provided (laptop/iPad, phone)Qualifications:Valid Michigan licensure as a Registered Nurse (RN), BSN preferredMinimum of 5 years of healthcare experience with hospice experience preferredPrevious leadership, management or supervisory experience preferredProficiency with Electronic Medical Records (EMR) systems and Microsoft Office applications.Responsibilities include:Oversee daily clinical services to ensure timely patient care and appropriate utilization of clinical resources.Monitor staffing coverage, visit completion, and workflow challenges, addressing operational issues promptly and effectively.Partner with Scheduling to maintain adequate staffing and optimize clinical productivity.Lead daily team prioritization and staff performance efforts to support quality care, service excellence, and productivity goals.Ensure consistent implementation of individualized care plans and adherence to hospice standards across interdisciplinary teams.Facilitate and lead Interdisciplinary Team (IDT) meetings.Promote accountability, timely care and communication, high-quality documentation, and professional practice.Conduct ride-alongs, shadowing, coaching sessions, and chart reviews to support professional growth and clinical excellence.Support staff through coaching, feedback, and ongoing performance improvement initiatives.Ensure clinical services are available 24 hours per day, 7 days per week, including participation in a manager on-call rotation and providing clinical support as licensure permits.Collaborate with physicians, the Medical Director, Director of Patient Care, and interdisciplinary team members to coordinate patient care and support patient and family goals.Build and maintain strong relationships with healthcare facilities, referral partners, and community stakeholders.Serve as an ambassador for Emmanuel Hospice, promoting our mission, services, and philosophy of care.Why Join Us?A mission-focused environment where your work has deep impactCompetitive salary with mileage reimbursementFlexible scheduling and supportive leadershipOpportunities for continuing education and professional growthComprehensive benefits package (medical, dental, vision, retirement)A team that values your compassion, professionalism, and heart
