Licensed Practical Nurse (LPN)

We're hiring a Licensed Practical Nurse (LPN) to serve as a primary provider of resident care in our Assisted Living or Memory Care community. This role reports to the Director of Health & Wellness. What You'll Do Provide direct resident care services and follow each resident's individualized plan of care Administer medications following standards of practice, state regulations, and required documentation Perform resident assessments as required by state standards and facility policy Observe residents for changes in condition and report immediately to the supervisor or Department Director Document observations, communications, and daily care by the end of each shift Complete and maintain individualized care plans; attend interdisciplinary care meetings Check and record vital signs, weights, and blood sugars as assigned Assist with supervision, coaching, and training of direct care staff Support fall prevention and restraint-free environment What You'll Need High school diploma or equivalent (GED) required Licensed Practical Nurse license in applicable state required Current CPR certification required At least 2 years of related work experience Knowledge of medical terminology, medical records management, and clinical/diagnostic procedures Physical Requirements Ability to sit, stand, walk, climb stairs, reach, turn, bend, stoop, crouch, kneel, and perform fine manipulation throughout the workday Ability to lift up to 25 pounds unassisted and to carry, transfer, push, pull, and reposition residents with assistance or equipment Normal or corrected vision and hearing, including color recognition Working Environment Work is performed in a well-lit, well-ventilated community setting, with occasional exposure to dust, fumes, odors, water, and noise Occasional outdoor exposure to weather and temperature changes Frequent interruptions and interaction with residents who may be emotionally distressed Flexibility to work beyond normal hours, on weekends, or in other roles as needed What We Offer Competitive pay Comprehensive benefits including medical, dental, vision, and life insurance Generous Paid Time Off (PTO) and Holiday Pay Flexible scheduling Excellent growth and advancement opportunities Additional Information This job description is intended to convey general information about the role and is not an exhaustive list of duties, responsibilities, skills, or working conditions.
