Responsibilities
- Provide comprehensive social services to patients and families across the lifespan to address physical, social, emotional, and environmental barriers to health.
- Conduct psychosocial assessments and develop individualized care plans in collaboration with patients, families, and the interdisciplinary team.
- Coordinate discharge planning, referrals, and follow-up resources to support safe transitions of care.
- Facilitate counseling and education for patients and families, and connect them with community supports as needed.
- Document clinical interactions, treatment plans, and outcomes in the medical record in accordance with policy and regulatory requirements.
- Collaborate with colleagues to ensure culturally sensitive care and effective team communication.

Requirements
- Master of Social Work (MSW) from an accredited school of Social Work
- Alaska license at the Master’s level (LMSW) required
- One year of experience in a hospital or health-care setting
- Valid Alaska State driver’s license
- Basic computer literacy
- Preference for cross-cultural experience with bilingual English and Yup’ik
- Proof of completed COVID-19 vaccination required
