Qualifications Registered Nurse License (Required) Hospice care experience: 1 year (Preferred) Full Job Description Hospital RN Case Manager at Anchor Health, a premier hospice organization, provides expert and compassionate care to patients and families facing terminal illness. The role involves coordinating hospice care plans, executing medical treatments, pain management, and providing education and support to patients and families. Essential Job Responsibilities Completes initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs, including a complete physical assessment and history of current and previous illnesses. Provides professional nursing care by utilizing all elements of the nursing process. Assesses and evaluates patient’s status, writes and initiates plans of care, and regularly re‑evaluates needs, revising plans as necessary. Uses health assessment data to determine nursing diagnosis. Develops a care plan establishing goals based on nursing diagnosis and incorporates palliative nursing actions, including family in the planning process. Initiates preventive and rehabilitative nursing procedures, administers medications and treatments as prescribed by physicians. Provides counseling to patients and families to meet nursing and related needs. Provides health care instructions and assists patients with daily living activities, promoting self‑sufficiency and optional comfort care. Acts as case manager, coordinating patient care for assigned caseload. Maintains accurate clinical notes and records pain/symptom management changes and outcomes. Communicates with physicians regarding patient needs, reports changes, and obtains orders as required. Coordinates with community health providers to align care plans. Teaches self‑care techniques to patients and families and provides medication, diet, and instructions as ordered. Maintains a safe environment for the patient. Supports family and interdisciplinary team to ensure continuity of care. Collaborates with family/caregivers and hospice interdisciplinary members to address emotional needs. Attends interdisciplinary group meetings. Position Qualifications Registered Nurse with a valid CA state license. At least one year of recent hospice experience with a strong foundation in acute care. Current CPR certification. Valid driver’s license, appropriate auto insurance, reliable transportation, and willingness to drive to patient locations. Excellent interpersonal skills and passion for hospice care.
