Care Advisor – Chronic Care Management Program Atlanta, GA Location State: Georgia Country: United States Position Profile We seek a compassionate and detail-oriented Care Advisor to join our Chronic Care Management (CCM) team. The ideal candidate will provide comprehensive care coordination and support to patients with chronic conditions, ensuring they receive optimal care while meeting quality metrics and program goals.
