Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

An opportunity is available for a clinical pharmacist with strong infusion and IV/TPN experience to support a busy program on a part-time basis (2-3 days per week). This role operates Monday through Friday from 8:30 a.m. to 5 p.m., providing a consistent weekday schedule in a collaborative environment.

Key Responsibilities:

• Review and evaluate physician orders for appropriateness including dosage, drug choice, potential interactions, and administration route.
• Maintain complete and current patient records including medical history, laboratory results, and medication profiles.
• Ensure compliance with documentation requirements for state, federal, and accrediting bodies.
• Coordinate with nurses and physicians on clinical laboratory results and patient medication therapy.
• Provide clinical and technical support to healthcare team members and patients.
• Assist with policy development and train pharmacy personnel including technicians and interns.
• Oversee sterile compounding processes and comprehensive quality assurance programs.
• Document communication with healthcare professionals and patients in medical records.
• Deliver drug therapy education and instructions to patients and caregivers.
• Uphold patient and staff safety by adhering to best pharmacy practices.

Qualifications and Desired Experience:

• Current, active California pharmacist license.
• Experience with IV, infusion, and clinical pharmacy practices; home infusion pharmacy background highly preferred.
• Familiarity with CareTend pharmacy system.
• Strong communicator and team player with positive attitude prioritized over years of experience.
• Recent graduates with IV/TPN exposure through APPEs or internships encouraged to apply, as training is available.
