• Provide direct patient care to residents in our post-acute rehabilitation and long-term care settings

• Administer medications, perform vital signs, and report changes in resident status to nursing staff

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Monitor and document resident progress, identifying areas for improvement and recommending revisions to care plans

• Collaborate with nursing staff, therapists, and other healthcare professionals to ensure seamless care transitions
