We are currently seeking a passionate and dedicated Speech-Language Pathologist (SLP) to join our rehabilitation team on a PRN basis .

Your Role as a Speech-Language Pathologist

As an SLP, you will provide skilled speech, language, cognitive, and swallowing services designed to help residents achieve greater independence and improve their quality of life.

Key Responsibilities

• Evaluate residents referred for speech-language pathology services.
• Assess communication, cognitive, speech, language, and swallowing needs.
• Develop and implement individualized treatment plans with realistic and measurable goals.
• Provide skilled therapy in accordance with physician orders and established plans of care.
• Treat residents with speech, language, cognitive-communication, and swallowing disorders as clinically appropriate.
• Monitor resident progress and modify treatment approaches based on clinical needs and response to therapy.
• Complete accurate and timely evaluations, daily treatment notes, progress notes, re-certifications, and discharge documentation .
• Educate residents, families, caregivers, and staff regarding therapy goals, strategies, and recommendations.
• Collaborate with nursing and the interdisciplinary team to promote coordinated, patient-centered care.
• Make appropriate recommendations regarding diet, swallowing safety, communication strategies, and functional needs within the scope of practice.
• Maintain confidentiality and comply with HIPAA, Federal Residents' Rights, and applicable regulatory requirements .
• Perform additional duties as assigned by the Director of Rehabilitation.

What We're Looking For

• Graduate of an accredited Speech-Language Pathology program .
• Current and valid Speech-Language Pathologist license/registration in the state of practice.
• Strong clinical, communication, documentation, and organizational skills.
• Compassionate and patient-focused approach to resident care.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Long-term care and/or geriatric experience preferred , but not required.
