Key Responsibilities of the RN – Case Management – Travel:

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers to implement individualized care plans
• Address and reassess cost-efficient care levels
• Ensure non-medical support compliance with treatment plans
• Coordinate care transitions as needed
• Engage specialty resources for optimal resolution
• Maintain detailed records of clinical, functional, and fiscal outcomes
• Identify health promotion and illness prevention opportunities
• Act as patient advocate, protecting privacy and confidentiality

Requirements of the RN – Case Management – Travel:

• Associates or Bachelor's Degree in Nursing or related field
• Current, unrestricted state Registered Nurse license
• Case Management certification
• Ability to analyze complex medical information and make rational decisions
• Experience with HCHB, Home Health, OASIS, and IV
• SC License/Compact licenses accepted
