Home Health Physical Therapist – Hood River, OR & Surrounding Areas

Schedule

• Start Date: ASAP
• Shift: 8-hour Days
  • 4x10s and 3x12s cannot be accommodated.
• Weekends: Not required
• On-Call: No
• Floating: No
• Block Scheduling: Not available
• Guaranteed Hours: Facility may call off up to one shift per pay period (every two weeks).

Experience Requirements

• Minimum 1 year of Skilled Home Health Physical Therapy experience required.
• First-time travelers are welcome to apply.
• OASIS experience is required. Please indicate if the traveler does not have OASIS experience.

Patient Load & Productivity

• 1:1 patient ratio
• Up to 6 patient visits per day
• 30 productivity points expected per day

Licenses & Certifications

• Active Oregon (OR) and Washington (WA) Physical Therapist licenses are both required.
• BLS (AHA) required.

Additional Requirements

• Traveler must have reliable personal transportation .
• Patient visits are grouped to minimize driving time; maximum travel radius is approximately 60 miles (60–90 minutes total drive time per day) .
• COVID-19 primary vaccination series plus one booster is required.

Local Candidate Policy

• Local candidates are accepted.
• Candidates residing within a 50–75 mile radius are considered local and are paid at the same rate.
