Work Environment

This position is located onsite at our rehabilitation facility in Bradenton, FL. Key Responsibilities

• Conduct thorough evaluations that include review of medical history, physical assessments, and formulation of patient-specific treatment plans.
• Effectively communicate evaluation findings and treatment strategies to both patients and their families, involving them in the goal-setting process.
• Administer treatment protocols based on clinical findings while continuously adapting approaches to align with patient progress and the evolving healthcare landscape.
• Collaborate with interdisciplinary teams by consulting with other healthcare professionals and documenting any changes in the patient's condition that may require modifications to the care plan.
• Manage discharge planning and ensure follow-up care is effectively communicated, maintaining accurate records of patient care activities that reflect the journey through therapy.
• Uphold productivity expectations through effective time management and by maintaining a well-organized schedule.
• Contribute to departmental objectives and support organizational compliance with regulatory and accreditation standards.
• Engage in ongoing professional development and actively pursue additional training to enhance competencies.
• Promote excellent patient experiences by demonstrating adaptability and responsiveness to patient needs.
• Participate in initiatives aimed at improving access to care while monitoring site-specific data to enhance service delivery.

Patient Population / Client Focus

As a Physical Therapist at Pathways, you will work with a diverse range of patients, including those recovering from surgery, injury, or chronic conditions. The clinic serves individuals of all ages, allowing you to develop a rich skill set in tailoring treatment plans to meet the specific needs of each patient, fostering their recovery and improving their quality of life.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution is required.
• A valid physical therapy license or eligibility for licensure in the state of Florida.
• Current certification in Basic Life Support (BLS) from the American Heart Association or equivalent.

Preferred Experience

• A minimum of 2 years of experience in a rehabilitative setting is preferred.
• Previous experience working in an outpatient clinic is advantageous but not mandatory.

Compensation and Benefits
