Job Title: RN Home Health Address: The Villages, FL Job Description Administer skilled nursing care for clients of all ages in their place of residence. Coordinate care with the interdisciplinary team, patient/family, and referring agency. Assess home health and/or hospice patients to identify their physical, psychosocial, and environmental needs. Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care. Educate patients and/or their families regarding care, diet, medications, and home safety. Supervise and provide clinical directions to HHA/STNA/LVN staff. Monitor assigned cases to ensure compliance with third-party payer requirements. Critical Information On-call responsibilities may be required as needed. Schedule: Shift is Monday through Friday until shift is complete. Education/Licenses Needed Current state license as a Registered Nurse is required. Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program is required.
