RN/LVN Pediatric Homecare

Job Overview Licensed Vocational Nurse (LVN) / Registered Nurse (RN) responsible for delivering skilled nursing care to assigned pediatric patients at home under limited supervision, following all applicable federal, state, and local regulations and agency policies. Responsibilities Provide skilled nursing care as ordered in the patient’s plan of treatment and nursing care plan. Document all care activities, including observations, interventions, and medication administrations, accurately in clinical notes. Notify the Director/Supervisor and Physician of new problems, changes in status, or unresolved issues as required by state regulations. Obtain necessary orders for interventions through physician notification. Observe aseptic technique during all procedures. Perform specific treatments and medication administration per the care plan. Act as a team member under the direction of the registered nurse. Communicate skilled observations and significant status changes to the Director of Clinical Services and/or Clinical Manager. Request modifications or additions to orders based on the patient’s clinical status and nursing judgment. Document all physician collaboration in the medical record. Utilize all available services appropriately and document their use. Participate in agency-mandated activities such as in‑services and meetings. Adhere to agency policies, procedures, and handbook requirements. Complete all clinical documentation accurately and submit it on time per agency policy. Represent yourself in a positive, professional manner within the company and the community. Maintain a clean, neat, professional appearance in accordance with the dress code. Report on time and adhere to scheduled shift times. Requirements & Qualifications Licensed as an LVN or RN in the state where services are provided. Graduated from an approved certification program or accredited nursing school (undergraduate or graduate level). Possess the professional experience required by state regulations for delivering pediatric home care. Complete all agency‑required screening and competency testing with a proficiency score of 80% or greater. Provide current TB test results or chest x‑ray documented by the agency. Provide current CPR certification. Have reliable transportation to travel to patient homes. Demonstrate flexibility, organization, and the ability to exercise independent, sound judgment. Physical Requirements Regularly sit, stand, bend, kneel, walk, balance, crouch, crawl, push, pull, and reach with hands and arms. Frequent lifting or moving up to 15 pounds and occasional lifting of 100 pounds or more if required by job duties. Maintain vision and hearing within normal limits, with or without aid. Perform anticipated level of physical assistance required for safe client care. Work Environment The role involves providing care in private homes, where lighting, acoustics, air quality, temperature, and environmental hazards may vary unpredictably. The employee may be exposed to wet or humid conditions, household pets or animals, fumes or airborne particles, and toxic or caustic chemicals. Travel to patient homes is frequent and required.
