Care Manager

Work Schedule Per Diem, varied schedule, work as needed. Days 0800-1630 including weekend coverage as needed. You will work at the UW Health 1415 E State St Clinic. In lieu of a BSN, an Associate Degree in Nursing (ADN) with 2 years of relevant equivalent experience will be accepted. Required Licenses & Certifications Current Registered Nurse licensure from the Illinois Department of Professional Registration. Required Care Management certification.
