Health Home Care Coordinator & Member Advocate
Postgraduate-Center-For-Mental-Heal
Postgraduate Center for Mental Health seeks a Care Coordinator to join the Care Coordination team in NYC. You will address member needs, provide care plan updates, and conduct outreach between visits within a Health Home model. You will engage with healthcare providers, community agencies, and families to ensure access to services, improve wellness, and reduce ER visits and hospitalizations.
